The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangement you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
SERVICES, FACILITIES AND MOTOR EQUIPMENT
Basic Services of Funeral Director and Staff $ 1,650.00
Our charge for this service includes twenty-four hour accessibility of staff and facilities to
respond to the initial request for service, consultation with the family, clergy, cemetery or
crematory, and other parties involved in the final disposition of the deceased; preparing,
filing, and securing notices, authorizations and permits, assistance with forms, and basic
overhead charges such as taxes, licenses, utilities and other necessary business expenses.
This fee for our basic services and overhead will be added to the total cost of the funeral
arrangements you select. (This fee is already included in our charges for direct cremations,
immediate burials and forwarding or receiving remains.)
Embalming $585.00
Except in certain special cases, embalming is not required by law. Embalming may be
necessary however, if you select certain arrangements, such as a funeral with viewing.
If you do not want embalming, you usually have a right to choose an arrangement that does
not require you to pay for it, such as direct cremation or immediate burial.

ALTERNATIVE SELECTIONS
Forwarding of remains to another funeral home $ 1,095.00
Our charge for this service includes transfer of remains to the funeral home, embalming,
basic services of funeral director and staff, overhead, and motor equipment to point of
transportation. This charge does not include viewing, ceremony, merchandise, transportation
or shipping charges. (Wood Base Air Tray – 150.00 additional; Combination Unit – 180.00 additional)
Receiving of remains from another funeral home $ 875.00
Our charge for this service includes transfer of remains to the funeral home, basic services of
the funeral director and staff, overhead and transfer of remains to place of disposition. This
charge does not include viewing, ceremony, graveside service or merchandise.
Direct Cremations available from $ 925.00 to $ 1,295.00
Direct Cremation $ 925.00
Our charge for this service includes local transfer of remains to the funeral home, shelter/refrigeration
of remains for up to 48 hours, basic services of funeral director and staff, overhead, necessary authorizations,
transfer of remains to the crematory, gathering and placing newspaper notices, securing certified copies of
the death certificate, assistance with disposition arrangements, and the cremated remains returned in a cardboard
temporary container.
The cremation fee, estimated charges for newspaper notices and health department charges for the required cremation permit and any requested certified copies of death certificate are additional and are due at the time of arrangements.
An additional fee of $90.00 per day will be incurred for refrigeration of the remains after 48 hours.
This may become necessary due to family schedules, or the availability of the physician and/or coroner to
complete the death certificate, which is required by Ohio law before a cremation can be performed.
If you want to arrange a direct cremation, you can use an alternative container.
Alternative containers encase the body and can be made of materials like fiberboard or
composition materials (with or without an outside covering). The containers we provide are:

This charge does not include embalming, dressing, cosmetizing, viewing, ceremony or graveside service, cash advance items or any additional merchandise such as an outer burial container which may be required.
Immediate Burial with Casket provided by purchaser $995.00
Immediate Burial plus Casket of your selection $995.00
Immediate Burial with Minimum Casket (Elderlite; grey cloth) $1,890.00
An additional fee of $90.00 per day will be incurred for refrigeration of the remains after 48 hours.
This may become necessary due to family schedules, or the availability of the cemetery to prepare the
gravesite for burial due to a weekend and/or holiday.

The following package selections are those most often requested, or we will create a special funeral package to meet your needs both emotionally and financially.
PACKAGE FEES FOR: SERVICES, FACILITIES, EQUIPMENT AND TRANSPORTATION
I. TRADITIONAL FUNERAL SERVICE $3,500.00
Includes: Local transfer of deceased to funeral home, basic services of funeral director and staff , overhead, embalming, other preparation of the deceased,
and use of facilities and staff for funeral ceremony or equipment and staff for
ceremony elsewhere, service/utility vehicle and funeral coach to cemetery or
crematory. Additional charge for use of facilities for viewing/visitation is
$125.00 per hour.
- With Four (4) hours viewing/visitation $ 4,000.00
- With Three (3) hours viewing/visitation $ 3,875.00
- With Two (2) hours viewing/visitation $ 3,750.00
- With One (1) hour viewing/visitation $ 3,625.00
II. GRAVESIDE SERVICE $3,200.00
Includes: Local transfer of deceased to funeral home, basic services of funeral
director and staff, overhead, embalming, other preparation of the deceased,
use of equipment and staff for graveside service, service/utility vehicle and
funeral coach to cemetery. Additional charge for use of facilities for
viewing/visitation is $125.00 per hour.
III. DIRECT CREMATION AND MEMORIAL GATHERING $1,550.00
Direct cremation ($925.00), and use of facilities and staff for one hour visitation ($125.00) and memorial service ($500.00). This total does not include any (required) merchandise, cash advances or sales tax.
